Naming note (2026-09): folders were renamed to RCA product names (platform/Platform_RTE, application/RBC_GP, ...). Entries below keep the repository names they were written with; see the layout table in CLAUDE.md.

Open defects only: real bugs found during work in this workspace, one entry each (found, symptom, investigated, status). Planned work is in TODO.dox. When a defect is fixed, delete its entry: the fix and its reason are in the commit message, and a review pass records its findings under safety_quality/.
